Am finding it difficult to differentiate scleroderma renal crisis from TTP. Do we just base it on history? tx is different.thanks.

Great question. This is a common clinical dilemma that physicians continue to face. Both can present with similar clinical findings: renal dysfunction, thrombocytopenia, peripheral schistocytosis. Here are some quick differences:

-- severe hypertension -- more common with scleroderma renal crisis (SRC)
-- diagnosis of scleroderma -- most patients with SRC will have this condition within the first 5 years of diagnosis. Individuals with long-standing scleroderma don't commonly experience SRC.
-- ADAMTS13 deficiency -- almost universally found in TTP versus SRC
