I have a question on his Plavix and a question about renal dosing sources.

I was reading about it, and saw no dose adjustment for renal impairment, but I did not find anything on folks with ESRD. Micromedex and up to date say no dose adjustment (I assume since only half the elimination is renal). Is there a good source about drugs in ESRD folk?

I believe clopidogrel (plavix) does not require dosing adjustment in renal disease. A great resource is an article by Aronoff that details the aspects of drug dosing in renal failure. He includes some simple mathematical calculations that can help practitioners determine how to adjust medications for patients with renal impairment. You can find it here.
